WaveFit Physical Therapy accepts Medicare and most PPO Insurances for therapy services. For specific information regarding coverage please contact our office and we will be happy to verify your individual coverage.
Upon arrival at our office, you will be asked to complete some basic paperwork. This paperwork is also available to complete online. Don’t forget to bring your insurance card(s) and your prescription for physical therapy and medication list. The therapist will then ask you a series of questions about your past and current medical conditions before performing a comprehensive evaluation. Based on your physician diagnosis, your therapist prognosis, and your goals an individual treatment plan will be created. You will leave your session empowered with professional advice and tips to live your best life.
